We partner with hospital discharge planners, social workers, case managers, psychiatrists, guardians, rehabilitation facilities, and community organizations to ensure smooth, timely, and compassionate placements for individuals who need a safe, structured residential environment.
We understand that the referral process can feel overwhelming – especially when someone is in crisis or needs placement quickly. Our team is here to make it as simple, fast, and supportive as possible. We respond to every referral inquiry within 24 hours.
We accept referrals for adults 18 and older who meet one or more of the following criteria:
– Diagnosed with a serious mental illness, including schizophrenia, schizoaffective disorder, bipolar disorder, major depressive disorder, PTSD, or other co-occurring conditions
– Transitioning from a hospital, crisis stabilization unit, or inpatient psychiatric facility
– Experiencing homelessness or chronic housing instability due to mental health needs
– Living with co-occurring mental health and substance use disorders
– Living with intellectual or developmental disabilities and a co-occurring mental health diagnosis
– Court-involved adults who require structured, community-based residential support
– Veterans or surviving spouses with behavioral health support needs
– Unable to live safely or independently without daily supervision and support
– Individuals whose family or caregiver can no longer safely provide the level of care they need
We accept the following funding sources:
– SSI (Supplemental Security Income) – Covers room and board for eligible individuals
– DAP (Discharge Assistance Planning) – Authorized through the local Community Services Board (CSB)
– Private Pay – Including VA Aid and Attendance or pension income for eligible veterans and surviving spouses
We understand that funding is not always finalized at the time of referral. In many cases, we can begin the admission process while funding is still being confirmed. Please contact us to discuss the specifics of any individual situation.

Once we receive a referral, we complete a brief confidential screening call, followed by a face-to-face assessment. If the individual is a good fit, we coordinate the care plan and move-in date - often within 24 to 48 hours.
