We serve adults 18 and older with serious mental illness, behavioral health needs, or significant support needs who require a structured residential environment. This includes individuals stepping down from hospital care, those experiencing homelessness due to mental health challenges, veterans, court-involved adults, and others who need more support than independent living can safely provide.
Stellaris Keystone Services, LLC is our licensed clinical and residential services provider. Stellaris Keystone Foundation, Inc. is our nonprofit community arm, providing free housing navigation, resource navigation, peer support, and community education. Together, the Stellaris Keystone family works to support individuals and families across every stage of their journey.
We respond to referral inquiries within 24 hours. Once a referral is confirmed and the necessary information is in place, placement can typically begin within 24 to 48 hours.
Yes. Our programs operate 24 hours a day, 7 days a week, 365 days a year. Our live-in caregivers are always present and on-site.

Yes. Transportation to medical, psychiatric, pharmacy, and community appointments is included in our residential program.
We accept SSI, DAP authorized through the local Community Services Board, and private pay including VA Aid and Attendance or pension income for eligible veterans and surviving spouses. Contact us if you have questions about a specific funding situation.
We offer CPR & First Aid Training, Home Health Aide (HHA) Training, Personal Care Assistant (PCA) Training, and Medication Administration Training programs.
Behavioral support involves structured, individualized strategies delivered by trained staff to help individuals identify triggers, build healthy coping skills, and replace challenging behaviors with positive, adaptive alternatives – always in close collaboration with the individual’s clinical team.
