We provide person-centered support services including Adult Services, Mental Health Sponsored Residential Services, Child & Family Services, Community Support, and future Home Care and Child-Placing Agency services.
Our Child-Placing Agency services are pending licensure and will become available after approval.

We complete a brief, confidential screening call to understand the individual’s needs and confirm that our program is a good fit. We want to make sure we can truly serve each person well.
We conduct a face-to-face assessment with the individual and, when appropriate, their family or treatment team. This allows us to understand their history, goals, and the support they need.
We develop a fully individualized care plan in collaboration with the individual, their family, and their clinical team. Every plan is tailored to the person – because real care is never one-size-fits-all.
Once everything is in place, services begin. In many cases, placement can happen within 24 to 48 hours of a confirmed referral, because we understand that waiting is not always an option.

Stellaris Keystone Foundation, Inc. is our nonprofit community arm, dedicated to removing barriers, restoring hope, and building stronger communities. We provide housing navigation, homeless outreach, resource navigation, peer support, and community education — completely free of charge to individuals and families across Culpeper, Fauquier, Madison, Orange, Rappahannock, and surrounding Virginia communities.

Street and community outreach, basic needs support, and documentation assistance.
SNAP, Medicaid, SSI/SSDI, utility assistance, transportation, and employment connections.
Peer support groups, family education, and peer mentoring for individuals and families.
